Collaborative Anesthesia Partners has partnered with critical access hospitals to provide Interventional Pain Management to our communities to help treat chronic or acute pain.
When you refer your patient to one of our providers, we will perform a history and comprehensive physical exam. Our providers may recommend physical therapy, lab work, imaging, injections, counseling and coping strategy therapy, and when needed, a surgical referral.
It is important to us that you are kept updated about your patient’s care. You can expect communication from the us regarding your patient’s treatment plan.
To refer a patient to one of our pain providers, please fax the following information to the applicable location:
- Patient Name and Demographics
- Copy of Insurance Card or Worker’s Compensation Information
- Referring Provider
- Reason for Referral and Supporting Office Note
- Imaging Studies (X-Ray, MRI), if applicable
Interventional Pain Management
Our goal for interventional pain management is to work in tandem with a team of providers that is specific to each patient to catalyze the treatments given in our office. Creating a team of interdisciplinary providers increases the success of an individual’s treatment plan and outcome. Each patient has a unique condition. Our pain providers will develop a plan just as unique by collaborating with the providers already working with each patient to address the cause while controlling pain.
